Step 1: Migrating Your Patient List
The foundation of any EMR migration is your patient roster.
Most EMR systems allow practices to export a complete patient list as a CSV file. This file typically contains demographic information such as:
Patient names
Dates of birth
Addresses
Phone numbers
Email addresses
Sex and gender information
Other contact and demographic details
Some EMRs allow administrators to generate this export directly, while others require assistance from your account representative or support team.
Once you obtain the CSV file, Akute will provide a secure cloud storage location where you can upload it. From there, our team imports the data and automatically creates patient charts within Akute.
This means that when you go live, your patient records are already established with their core demographic information in place, eliminating the need for manual chart creation.
Step 2: Transferring Clinical Records
Beyond patient demographics, practices also need access to clinical information, historical documentation, and patient records.
Most EMRs can export clinical data using an industry-standard XML format known as a Continuity of Care Document (CCD). These exports are often organized into individual folders for each patient and may include:
Clinical summaries
Problem lists
Procedure histories
Family history
Social history
Lab reports
Imaging results
Forms and documents
Faxed records
In many cases, the exporting EMR packages these folders into a single compressed ZIP file.
For EMRs that do not support CCD exports, the alternative is often a collection of PDF chart exports organized by patient.
Once you receive these files, Akute provides a secure location for uploading the export package. Our team then associates the files with the appropriate patient charts and begins the migration process.
What Clinical Data Can Be Imported?
The answer depends largely on the structure and quality of the data provided by your previous EMR.
When structured CCD data is available, Akute can often import key clinical information directly into the chart, including:
Problem lists
Family history
Social history
Procedures and surgical history
Other structured clinical data elements
This allows providers to work with searchable, usable information rather than simply reviewing archived documents.
Understanding the Limitations of Healthcare Data Interoperability
One of the biggest misconceptions about EMR migrations is that all healthcare systems store and export data in the same format.
Unfortunately, that isn't the case.
Healthcare interoperability has improved over the years, but every EMR vendor structures data differently. As a result, certain information may not always be available in a format that can be imported directly into another system.
For example, provider notes are frequently exported only as PDF documents rather than structured data. When this happens, Akute can preserve and attach the notes to the patient chart, but cannot automatically convert the contents into editable or searchable note entries.

When these data elements are available, our team works to import them whenever possible. However, the completeness of the migration ultimately depends on what the source EMR provides.

Medication and allergy data can be particularly inconsistent across EMR exports.
Even when a prior system cannot provide this information in a structured format, Akute offers alternative ways to rebuild medication histories. What Happens to Data That Can't Be Imported?
Not every piece of clinical information can be converted into structured fields within a new EMR.
Rather than leaving important information behind, Akute preserves those records and makes them accessible within the patient chart.
